When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment.
Your initial exam help us get acquainted with each other and learn about your goals and desires for your dental treatment.
Generally, X-rays will be taken allowing your doctor to examine the jaws structure, teeth position and teeth decay. Oral hygiene instructions will also be provided along with suggestions to help you care for your teeth. In most cases, we will also clean your teeth on this visit and provide an evaluation that will outline your existing dental problems and proposed treatment.
After your treatment plan is prepared we will require a second or more appointment to provide treatment on another day. Please assist us by providing the following information at the time of your first visit:
- Submit your New Patient Forms online before appointment
- Provide us with your dental insurance information before appointment. This will save time and allow us to help you process any claim.
- A list of medications you are presently taking.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Our office utilizes digital radiography. This system allows us to take the necessary radiographs in order to diagnose your Dental condition. Your previous dentist may have taken radiographs in the past. Unless your previous dentist could send us your most recent diagnostic quality radiographs to our office prior to your visit we will take the necessary radiographs in order to diagnose your current conditions. Some radiographs that we see from other offices do not allow us to accurately diagnose your current conditions.
It is vital that we have diagnostic quality radiographs. If we do not have the proper radiographs to properly diagnose your current dental conditions, we will take the necessary radiographs in order to formulate the necessary treatment plan to address your dental health.
Our radiographic policy is to take the necessary radiographs to accurately diagnose the possible presence of dental disease. Regardless of what your insurance will or won’t cover or what radiographs may have been taken in the past we will take what is necessary for proper diagnosis. Your treating Dentist will determine what radiographs he or she will need for diagnosis.
